The short answer
People eating a vegan diet need a reliable source of vitamin B12 from fortified food, a supplement, or both. Fruit, vegetables, grains, legumes and ordinary fermented foods do not reliably supply active B12. Health New Zealand advises daily B12-fortified foods[1] and says a supplement may be needed when these are not eaten regularly. Pregnancy, breastfeeding, neurological symptoms or suspected malabsorption require prompt clinical advice.

How much do the reference values say?
Australia and New Zealand set[3] an adult RDI of 2.4 micrograms per day, increasing to 2.6 micrograms in pregnancy and 2.8 micrograms in lactation. These values describe absorbed needs and total dietary intake for generally healthy populations. They are not the same as a treatment dose.
Supplement labels often contain amounts far above the RDI because absorption from a single oral dose is limited and partly passive. That does not mean the biggest number is best. Frequency, formulation, adherence, absorption and clinical purpose matter.
Do the fortified-food maths
Health NZ lists some plant milks, breakfast cereals, savoury yeast products, yeast extracts and meat alternatives as possible fortified sources. “Possible” is important: recipes change, and two products in the same category may differ.
Use this method:
- Find vitamin B12 on the nutrition panel.
- Confirm whether the value is per 100 mL/100 g or per serving.
- Measure the serving you actually use.
- Add all servings across the day.
- Recheck when the package changes.
For example, if a plant milk declares 0.8 micrograms per 250 mL and you use 125 mL, that contributes 0.4 micrograms. If a cereal declares 1.0 microgram per 40 g but your bowl is 30 g, it contributes 0.75 micrograms. This fictional arithmetic teaches label reading; it does not validate those foods or establish a complete daily plan.
Foods that are not reliable B12 insurance
Health NZ says seaweed, algae and fermented foods often contain forms the body cannot use. Spirulina should not be relied on as a B12 source. Mushrooms provide only a small contribution and do not replace fortified foods or a supplement in a vegan plan.
Unverified “natural B12” marketing is particularly risky because standard assays can count inactive analogues. The useful question is whether a product contains a recognised active vitamin B12 compound in a declared amount.

Cyanocobalamin is stable and well studied. Methylcobalamin is also biologically active, but claims that it is universally absorbed better or “detoxifies” are not established. Choose from evidence, dose reliability and practicality rather than a biochemical-sounding label.
Liquid products require an extra check: amount per drop, per dropper and per millilitre can differ. Do not assume a full dropper equals one dose.
Pregnancy, breastfeeding and infants
Health NZ specifically identifies breastfed infants of vegan mothers as at risk. An infant starts with limited stores, and maternal intake/status affects breast-milk B12. The Australia and New Zealand NRV advises vegan mothers to supplement at the RDI level throughout pregnancy and lactation, but an individual maternity plan should check the actual product and any existing deficiency.
Do not adapt an adult tablet for a baby or wait for developmental concerns. Discuss maternal B12 early with a midwife, GP or dietitian and use paediatric advice for the infant. Formula and fortified complementary foods have separate standards.
When testing matters
A clinician may order a full blood count and serum B12. When results are borderline or symptoms suggest deficiency, methylmalonic acid, homocysteine or tests for pernicious anaemia may add context. Folate can correct some blood-cell changes while neurological B12 injury continues, so a high-folate supplement should not be used to bypass assessment.
Reasons for low B12 include low intake, pernicious anaemia, coeliac or Crohn's disease, stomach/bowel surgery and medicines that affect absorption. Treatment differs by cause. Health NZ notes that serious malabsorption may require injections or high-dose oral treatment; these are clinical regimens, not wellness doses to copy.
Frequently asked questions
Are B12 supplements vegan?
Health NZ says the B12 ingredient in supplements is not made from animal products. Check the complete product, including capsule shell and flavourings, if vegan certification matters to you.
Can nutritional yeast provide enough?
Only if the exact product is fortified and the amount from your real serving fits a reliable plan. Unfortified yeast is not a B12 source.
Does a normal blood count rule out deficiency?
No. Neurological effects can occur without classic anaemia, and folate intake can alter the blood picture. Symptoms require clinical assessment.
Should I choose a B-complex instead?
Not automatically. A stand-alone product makes the B12 dose easier to see and avoids unnecessary amounts of other B vitamins. Compare your actual need and every overlapping supplement.
How quickly does B12 work?
That depends on the cause, severity, symptom and treatment route. Do not use an expected timeline to self-diagnose or delay review of neurological symptoms.
